For tax purposes and other legal reasons you will need to contact the Social Security Administration.  The bride must file an SS-5 form (application form for changing name and /or marital status) within the fiscal year.  Even if the bride is keeping her maiden name the SS-5 form must be filled out.  An original certified copy of the marriage license from the County Clerk's office where the marriage license was obtained, must be presented with the SS-5 form.

It would be a good idea to purchase two original certificates because Social Security may want to keep one.  Call your local Social Security office to see how to present the form and certificate to them, mail or walk in and if they can return the certificate to you.  If they can return it to you then probably one certificate will be all you will need.

Do not try to make any changes until you receive a certified marriage license from the County Clerk's office.  Go to http://www.ssa.gov/online/ss-5.pdf  for further information and to print a copy of the SS-5 form.  There are five pages to print.  The last page is the one you will need to fill out.  The other four pages are instructions.  This form is provided free of charge.
